Patient Counseling Rubric for Evaluation

This rubric is designed to be used as a tool for enhancing patient counseling performance and providing consistency between instructors’/preceptors’/supervisors’ grading/evaluation.  It is a dynamic, working document for guidance only.

Ideally, this rubric would be used in conjunction with the Patient Counseling Evaluation form also available in the tools section of the DELTA Rx Institute website.

Note: This rubric is designed to be used as a tool for enhancing patient counseling performance and providing consistency between instructors’/preceptors’/supervisors’ grading/evaluation.  It is a dynamic, working document for guidance only.  Because of the difficulty in grading an oral exchange while it is occurring, this is only a guideline for both student/intern/pharmacist and instructor/preceptor/supervisor.  Grades/evaluation will be assigned by instructor/preceptor/supervisor using their overall impression while using this rubric to provide feedback.

 

 

1.  Identifies self properly; Purpose of session explained

 

1. Skips both

 

2. Skips one or the other; only introduces self with name (“Hi, I’m Linda.”)

 

3. Introduces self (name and job title (pharmacist or technician)(“Hello, I’m Linda, the pharmacists who filled your prescription today.”); Asks permission to discuss  (“Do you have a few minutes to discuss your prescription?”)

 

4. Introduces self (as above), Asks permission to discuss AND explains why time is needed (purpose/importance of counseling session).

 

5. Introduces self (as above), Asks permission to discuss AND explains why time is needed (purpose/importance of counseling session); Verifies who is the patient; Lays groundwork for conversation. (“There are 3 important things you need to know before taking this medicine.”  “There is some important information you need to know on how to take this drug safely and have it work well for you.”)
